I am sorry for the delay in answering your message. I am confused by the fact that your headaches come in the middle of the night, yet you feel better laying down without your pillow. Usually the pillow would not make a huge difference in a spinal headache. However, if indeed you lay completely flat and consistently have no headache when you do, this would be in favor of a spinal headache origin. Most spinal headaches are not episodic, they are present daily.

You might want to bring up the possibility with a neurologist or neurosurgeon. If you back is tough to get into, perhaps a flouro-guided blood patch would be better as test to see if your headache indeed resolves.
