I am not sure if I am missing something, so to save a phone call...........

Do I have to mail the ORIGINAL Optigen result form to OFA to get it posted on the site? I do like to keep originals for my records, just a thing........

I also could not find info as to is there an OFA form to submit and the fee schedule?

If someone, here, can give me a hint I would be grateful.

I did it a year ago, but I faxed it in and it was $15.00 credit card. I think I called and they told me what to do
