You earn BISS (Best In Specialty Show) but going Best In Specialty Show. Not sure what you mean by how do the group placements work, but unless a specialty is held in conjunction with an all breed show, there are no groups. If it is held in conjunction, than the BISS would go on to represent their breed in that group.

At an all breed show, the best of breed of each breed competes in their group - Labs are in the sporting group. Currently there are 7 groups, but AKC was/is changing things around - not sure the status of that change. But there are 4 group placements, but the dog who wins their group goes on to compete in Best in Show (BIS). There is only one placement here - the top one.
You can read all about how shows work by reading the rules available on the AKC website.
